Support After Hospital
If you’ve had help from the Home Not Hospital service, we’ll still be here for you afterwards. We want to make sure you have all the support you need to get back on your feet and feel confident at home again.

We welcome referrals from a variety of
organisations, professionals, and individuals.
Referrals can be made by:
Healthcare Professionals Doctors, nurses, therapists, and other healthcare professionals who believe that our support could benefit their patients.
Social Workers or Community Support Workers Professionals who support adults and families in Derbyshire with various needs.
Housing Associations Organisations that provide housing services to individuals and families.
Local Authorities Authorities responsible for providing public services and support within Derbyshire.
Family Members and Caregivers Relatives or caregivers who are concerned about the well-being of their loved ones and believe they could benefit from our support.
We also accept self-referrals from adults in Derbyshire who believe they could benefit from our services.
